Question 1

Type your uniqueclient identifier (UCI), ifknown.Otherwise, leave it blank. If this is your first time dealing withIRCCyou willnothave aUCI.

Type your family name (surname) exactly as shown on your passport ortravel document (even if the name is misspelled). Do not use initials.

Nick names or Alias

   This means that you are officially separated and have legally endedyourmarriage.

   This means that you and your spouse have had a ceremony that legallybinds you to each other. Your marriage must be legally recognized inthecountry where it was performed and in Canada.

   Separated

   This means that you are married, but are no longer living with yourspouse.

   Single

   This means that you have never been married and are not in acommon-lawrelationship.

   Widowed

   This means that your spouse has died and that you have notre-married orentered into a common-law relationship.

2. Enter the date (year, month and day) you were married or you entered into yourcurrent common-law relationship.
3. Type the family names and given names of your current spouse or common-lawpartner.

If you are in a common-law union, you must also fill out the StatutoryDeclaration of Common-law Union [IMM 5409] (PDF, 863 KB) form andinclude it with your application. If you are married, you must sendaphotocopy of your marriage licence or certificate with your application.

Ask your employer if you’re not sure of which option you should choose

• Labour Market Impact Assessment Stream; or
• Exemption from Labour Market Impact Assessment.

If you choose “Open Work Permit”, leave questions 2 to 6 blank.

If you’re applying for a post-graduation work permit,

• choose “Open Work Permit”;
• write “post-graduation work permit” in question 2a;
• leave the following questions blank: 2b, 3, 4, 5 and 6.

Do not select “Other” unless your situation appears on the listbelow.

Who should select “Other”?

Choose “Other” only if you’re applying to workinCanada for a:

• foreign mission or international organization recognized under the ForeignMissions and International Organizations Act;
• foreign government; or
• bridge or tunnel authority.

Your employer must give you an employment contract or an employment letterincludingthe following information:

• name of employer;
• address of employer;
• job title;
• salary;
• hours of work;
• any other relevant details.

Your employer does not need to submit an offer of employmentthroughthe Employer Portal. This means you don’t need to provide a labour market impactassessment (LMIA) number or offer of employment (LMIA-exempt) number.

a) If you need an LMIA to get a work permit, type the name of the employer as shownonthe LMIA. Attachoriginalemployment contract and LMIA.

If you do not need an LMIA to get a work permit, type the name of the employer whogaveyou the offer of employment number.

If you are applying for a post-graduation work permit, write “post-graduation workpermit” as the employer name.

Question 5

Type the expected start and end dates of your work in Canada(From–To).Use format YYYY-MM-DD.

The dates should match the dates in either the:

• labour market impact assessment (LMIA);
• offer of employment; or
• duration allowed in the work permit category, if applying for an open workpermit.

For example, the “From” and “To” date for a spousal open work permit must match thevalidity of the principal applicant's permit.

Note:A work permit cannot be issued for a duration that islonger than the validity of your passport.

Question 6

Type the seven-digit number of your valid LMIA or the alpha numeric combination oftheoffer of employment number (LMIA Exempt). This number begins with the letter “A” andisfollowed by seven numbers.

Check the box to tell us if you have any post-secondary education (including university,collegeor apprenticeship training).

Examples of post-secondary education:

Trade or Apprenticeship

Training completed in a specific trade, such as carpentry or auto mechanics.

Non-university certificate or diploma

Training in a profession that requires formal education but not at the universitylevel(for example, dental technician or engineering technician).

Bachelor’s degree

Academic degree awarded by a college or university to those who finished anundergraduatecurriculum; also called a baccalaureate. Examples include a Bachelor of Arts,Science orEducation.

Master’s degree

Academic degree awarded by a graduate school of a college or university. You musthavecompleted a Bachelor’s degree before a Master’s degree can be earned.

PhD

Highest university degree usually based on at least three years of graduate studiesand athesis. Normally, you must have finished a Master’s degree before a PhD can beearned.

Previous activity or job

Give details of your previous activity or job for the past10years. If you are retired, include the details about the 10yearsbeforeyour retirement.

If you need more space, print out another page of the form, fill in this section andsubmit it with your application.

You must answer all questions in this section, otherwise your application will beconsidered incomplete and will be sent back to you.

1. you or any of your family members have ever had tuberculosis of the lungs orhavebeen in close contact with a person with tuberculosis within the past two years.
2. you have any physical or mental disorder that would require social or healthservices other than medication during your stay in Canada.
